CREATE INDEX CONCURRENTLY in a migration: turn off the transaction, add IF NOT EXISTS

On Postgres, CREATE INDEX locks writes to the table while the index builds. On a big table that's minutes of blocked inserts during your deploy. CONCURRENTLY avoids the lock, but it can't run inside a transaction, and Laravel runs each Postgres migration inside one.

return new class extends Migration
{
    public $withinTransaction = false;

    public function up(): void
    {
        DB::statement('CREATE INDEX CONCURRENTLY IF NOT EXISTS posts_source_published_idx ON posts (source_id, published_at DESC)');
    }

    public function down(): void
    {
        DB::statement('DROP INDEX CONCURRENTLY IF EXISTS posts_source_published_idx');
    }
};

Why IF NOT EXISTS: if the deploy times out halfway, the migration isn't recorded as run, and the retry would fail on the existing name. Also check pg_index.indisvalid after a failed concurrent build, because Postgres leaves an INVALID index behind that you have to drop.

One more Postgres detail: a new index often isn't used for index-only scans until VACUUM has updated the visibility map, so run VACUUM (ANALYZE) posts; after building it on a large table.
