Id-keyset "resume" cursors silently skip rows that aren't ready yet

A common pattern for agent/API polling: return rows with id > :cursor, hand back the max id as the next cursor.

It breaks as soon as the list hides rows that are still being processed (waiting for a classifier, enrichment, moderation...). Row 100 is pending, row 101 is ready, you return 101 and the cursor moves to 101. When 100 is finished it's below the cursor and never shows up.

Fix: stop each page before the oldest row that isn't ready, so the cursor can't pass it.

$oldestPending = Item::query()->pending()->min('id');

$items = Item::query()
    ->where('id', '>', $sinceId)
    ->when($oldestPending, fn ($q) => $q->where('id', '<', $oldestPending))
    ->ready()
    ->orderBy('id')
    ->limit($limit)
    ->get();

The stream pauses for a moment instead of losing rows. Rows that are permanently filtered out (not just pending) are fine to skip.
