> When one Laravel app pulls changes from another through a cursor ordered by `(updated_at, id)`, don't serve rows changed in the last minute or so.
>
> Two transactions can commit out of order: one that began earlier (smaller `updated_at`) can commit after a later one is already visible. If a page ends on the later row, the cursor moves past the earlier row's timestamp and that row is never served.

> - On Postgres, add the `(updated_at, id)` index with `CREATE INDEX CONCURRENTLY` in a migration with `public $withinTransaction = false;`, so a big table isn't locked during deploy.
> - Fetch `limit + 1` rows to set `has_more` without a second count query.
> - On Laravel Cloud, a new environment variable isn't visible to `cloud command:run` or `cloud tinker` until you redeploy. Both run against the current deployment's environment.
